Payment
The non-refundable Home Visitor Assessment Application fee of $325 must be submitted with your application. Make sure to secure funds before you are ready to submit your application. Learn here about CDA Scholarships in your state.
Requirements
The Home Visitor Assessment Application will ask you to
- Confirm that you meet eligibility requirements.
- Confirm that you completed all prerequisites.
- Obtain Advisor Statement to verify the observations were conducted according to Council standards.
- Obtain your Program Director signature as a consent that you may participate in the LAT Meeting.
- Collect Parent / Community Representative Statement to verify that all assessment steps were conducted according to the Council standards.
- Sign a statement of ethical conduct.
- Include documents verifying your training.
- Include the $325 Direct Assessment Application fee.

Application Deadline
The Council accepts Home Visitor Assessment Applications on a rolling basis. Your assessment will occur within 90 days from the date the Council receives your complete and correct application.

If you followed the process and correctly filled out the application, you are ready for Assessment. Proceed to STEP 4.
TIPS & TOOLS
- To avoid any delays, check your Home Visitor Assessment Application for any mistakes or omissions before mailing it to the Council. In particular, please make sure:
(1) the application is filled out correctly and entirely
(2) the persons who observed your work correctly completed the appropriate sections of the application
(3) the application, assessment fee / proof of payment, and training documents all arrive in the same envelope – please, do not mail any other documents to the Council

Glossary
- Assessment – collection of the evidence of your skills
- Local Assessment Team Meeting – the meeting of the Local Assessment Team where Team members share information, discuss, and evaluate the CDA Candidate's competency in the 13 Functional Areas. They make a final recommendation about whether the Candidate should be awarded the CDA Credential.
